For Faculty and Staff-

University Health Services is pleased to announce that beginning January 5, 2008 we will be offering appointments for acute illness and injury to faculty and staff. To schedule an appointment, call the health center at (330) 672-2322. We are open for faculty/staff appointments from 8:00-5:00 Monday through Friday. Appointments are limited, but usually can be accommodated the same day or the next day.

The KSU Faculty/Staff Convenient Care Clinic is staffed by board-certified physicians and nurse practitioners who are trained to diagnose, treat and write prescriptions for a variety of common acute illnesses. We are available to treat colds, flu, pinkeye, sore throats, ear and sinus infections, rashes, skin infections, cold sores, minor injuries, etc. We do not provide faculty and staff with care for chronic medical conditions or complex medical problems. If you have a chronic health problem or have been experiencing symptoms for an extended period of time, please schedule an appointment with a primary care provider in the community.

We encourage all employees to have a primary care provider in the community. If you do not have a primary care provider, go to www.mmoh.com or www.anthem.com to choose a physician in your area.

Faculty and Staff must print and complete the medical history form and privacy statement below prior to their scheduled appointment. Then the completed form(s) must be brought to the health center at the time of appointment.

UHS Class Excuse Policy

Kent State University Health Services recognizes how important it is for students to attend classes; therefore, it is our policy to provide a medical excuse only for those students who, in our medical opinion, need to stay at home and rest. Students who are very ill or highly contagious may be given a written note explaining when and how long we recommend they should be excused from classes. These students will still be responsible for contacting their professors and making up any missed assignments/tests. The physician or nurse practitioner treating the student will be the only individual to decide if an excuse note is to be given based on the student’s diagnosis. We will not write excuses for a past illness or for an illness/condition that we did not treat. The walk-out statement (receipt) that students receive after reconciling their bill at the cashier’s office is not meant to be used as a class excuse. It is simply verification that the student was at the health center.
