The Medical Records Department will
systematically and legally maintain a confidential,
accurate and standardized chart for every person
receiving medical services at the health center.
These charts will be protected from loss, alteration,
destruction and unauthorized and inadvertent disclosure.
General Facts about Medical
Records:
Medical records are maintained in
a secure electronic format and conform to all state
and federal regulations for the storage of medical
data. University Health Services maintains an accreditation
by the AAAHC and follows all recommended medical
records standards.
If you are age 18 or over, you are
responsible for your own medical information. Only
you can give us permission to release your medical
information to another person; this includes you
parents, your family doctor, or your professors.
We cannot give out copies, or talk on the phone
about your information without your consent, except
in exceptional situations prescibed by law. You
have the right to view your record or have it amended.
