Immunization Information

MMR Requirement:
Current immunization policy requires that
students attending the main campus must provide
proof of the following immunizations:
1. Documentation of two MMR vaccines given after
one year of age and separated by at least one month
OR
2. Documentation of one MMR vaccine within the last
six years.
Methods of Documentation:
1. Request a copy of the record from your high school
OR
2. Copy the page from your baby book containing
MMR dates and write name and SS# on it OR
3. Download the immunization form for your healthcare
provider to complete
Mail, fax, or bring the records to University Health
Services. DO NOT enclose the records with any other
correspondence to the university. Be sure to include
student's name and SS# on the record.
The following students are exempt from the MMR
requirement and are NOT required
to complete the immunization form:
1. Students born before January 1957
2. Students who attended classes on the main campus
before fall semester 1996
3. Students graduating from an accredited Ohio high
school since 1993*
*ALL transfer and graduate students (including Ohio
graduates) are required to submit the immunization

TB Requirement (International Students Only)
Students from the following countries are required
to have a tuberculosis screening test (TB test –
Mantoux type): All countries of Africa, countries
in Eastern Europe, Russia, Mexico, countries of
Central and South America, the Carribbean island
nations and territories, and all Asian nations with
the exception of Japan. This test must be completed
and results submitted to UHS within 12 months prior
to starting classes. Please download and submit
a completed immunization form OR submit a copy of
the TB screening information from your healthcare
provider.

Meningitis Disclosure for Residence Hall
Students Only:
Meningitis & Hepatitis B Vaccinations:
Effective July 1, 2005 any student living in on-campus
student housing will disclose vaccination status
information regarding Meningitis and Hepatitis B
through the Residence Hall application process.
University Health Services DOES NOT require additional
documentation of Meningitis and Hepatitis B vaccinations.
